Radiographic studies on caries progression at

approximal surfaces in adolescents and young adults

Abstract –

Along with the caries decline among children and adolescents in many western industrialised countries it has also been observed that in general, lesion progression is a slow process. As a consequence, the timing for operative intervention has changed, and non-operative measures to stop lesion progression rather than drilling and filling has become a common philosophy. As for approximal surfaces, it is commonly advocated not to restore until the lesion is observed in the dentin. The knowledge about lesion progression in the dentin is, however, scarce. The aim of the Västerhaninge study was therefore to assess the rate of lesion progression, particularly in the outer half of the dentin and to identify factors that influence the rate of progression. It was concluded that, in general, caries progression is a slow process. The deeper the lesion, however, as judged radiographically, the faster the rate of progression. Other influencing factors are the type of surface, caries activity, and age of the individual. Furthermore, preparation damage from drilling of the neighbouring surface is a risk factor for lesion progression.

Incomplete removal of carious lesions. Long-term evaluation

at microbiological and clinical levels.

Abstract –

One important step in the tooth restoration process is the removal of carious dentine. According to the traditional dentistry, all infected carious dentine has to be removed in order to stop caries progression. Bacteria beneath restoration are believed to be one reason for failure of restorations. However, studies of stepwise treatment demonstrated that, after incomplete removal of carious dentine and tooth sealing, the number of remaining bacteria is significantly reduced. Furthermore, the dentine underneath the provisional restoration acquires the characteristics of an inactive caries lesion (it became dry, harder and darker). The remaining carious dentine can be completely removed after reopening the tooth. The need to reopen the tooth to remove the remaining carious dentine is not well established. Nevertheless, it is conventional wisdom that all soft dentine should be removed before tooth restoration. Already in the 50s and 60s the presence of bacteria was normally identified beneath restorations following conventional caries removal procedures. It is not clear if bacteria beneath restorations represent any danger for their longevity. Studies on long-term evaluation of incomplete removal of carious dentine at microbiological and clinical levels will be presented.

Clinical studies on the onset, development and arrest

of occlusal caries in children and adolescents

Abstract :

The combination of intensive research activities on dental caries and the experience on caries decline obtained during the past few decades has provided a deeper understanding of the measures applied to caries management. However, the diagnosis and control of occlusal caries progression remain major problems in the clinic as well as in the international scientific community. The occlusal surfaces of posterior teeth are the sites within the dentition, which are most frequently attacked by the disease. The onset already takes place at the early stages of tooth eruption. Methods applied to control occlusal caries progression in children and adolescents will be presented and discussed in daily practice.
